Offering care-experienced young people support through education, training and employment

This service is focused on improving the pathway outcomes to education, training, and employment for care experienced young people aged 16-19 who are or were Looked After at Home (as well as those who are in kinship care, fostering, residential etc). We will also identify and engage those more difficult to reach young people who have left the care system aged 19-29 and are no longer being tracked by services.

Discovering Your Potential is a partnership between Action for Children, Barnardos, and the Prince's Trust.

Our approach

We offer intensive support to care experienced young people with the most complex needs. This longer-term support will help them move to positive destinations. We work with a case management approach, meaning that each young person will be allocated a designated worker who will work closely with the young person, social work teams, and partner agencies to identify and overcome personal and practical barriers into employment.

How it works

Within the service, participants will be supported with topics such as:

  • Personal Support
  • Employability Sessions (CV & Interviews, Sourcing Opportunities Job/College Search & Applications)
  • Engagement and assessment-individual service plan and goal setting Support to Move into Positive Destinations

All participants will be allocated a key worker who will support them throughout the 1:1 process. To ensure sustainable outcomes are achieved, these key workers can provide further support for up to 6 months depending upon the individual needs.
