A service for families of children and young people with disabilities and special needs

Many families caring for a children or young person who is disabled or has special needs receive help and support from health, education and social services departments. However, parents do not always receive clear information about the services that are available to them and may not know who to contact to request the services that and their child/young person need.

Hawthorn Project was set up as a Pembrokeshire Children’s Centre in August 2000 to address this problem by improving inter-agency cooperation and offering support and advice to families. The centre works in partnership with Pembrokeshire County council and Hywel Dda NHS Trust.

We collect information on all kinds of issues related to caring for a child or young person with special needs. You can call in or telephone with queries and if we haven’t got the information you need, we will find it for you.

Prosiect Hawthorn

Gwasanaeth ar gyfer teuluoedd plant a phobl ifanc gydag anableddau ac anghenion arbennig yw hwn.

Mae llawer o deuluoedd sy'n gofalu am blant neu bobl ifanc sydd ag anghenion arbennig neu sy'n anabl yn cael help a chymorth gan yr adrannau iechyd, addysg a gwasanaethau cymdeithasol. Ond nid yw rhieni'n cael gwybodaeth glir am y gwasanaethau sydd ar gael iddynt bob amser, ac efallai nad ydynt yn gwybod â phwy i gysylltu i ofyn am y gwasanaethau sydd eu hangen ar y plentyn/person ifanc.

Sefydlwyd Prosiect Hawthorn fel Canolfan Blant Sir Benfro yn ôl ym mis Awst 2000 i fynd i’r afael â’r broblem hon, a hynny drwy wella’r cydweithio rhwng asiantaethau a chynnig cymorth a chyngor i deuluoedd. Mae’r ganolfan yn gweithio mewn partneriaeth â Chyngor Sir Penfro ac Ymddiriedolaeth GIG Hywel Dda.

Byddwn yn casglu gwybodaeth am bob math o faterion sy’n ymwneud â gofalu am blentyn neu berson ifanc ag anghenion arbennig. Gallwch alw heibio neu ffonio gyda’ch cwestiynau. Os nad yw’r wybodaeth gennym, byddwn yn dod o hyd iddi.
